I declare that all data provided by me in the application form is true, and the attached photos show my real image. I consent to the processing of my personal data and image by Anna Guzior-Rutyna (Swatka Anna) and her authorized associates and employees - exclusively for the purposes of this matrimonial casting and subsequent ones.

I also consent to:

• Verification of the information I have provided, conducted by Anna Guzior-Rutyna and her associates.

• Sharing my image and information contained in the form with the person for whom the casting is organised - excluding my surname and residential address.

I acknowledge that:

• Anna Guzior-Rutyna undertakes to maintain complete discretion and not to share my image or personal data publicly.

• The data is processed in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council (GDPR) of April 27, 2016 on the protection of natural persons with regard to the processing of personal data.

• I have the right to access and correct my data, restrict its processing, or withdraw my consent at any time.

I consent to receiving commercial information regarding offered services, products, events, training, consultations and other paid services related to the activities of Swatka Anna via e-mail, SMS or telephone contact.

I also consent to receiving information about commercial offers, promotions and services of business partners cooperating with Swatka Anna. My personal data, in particular my e-mail address and telephone number, will not be shared with these partners. Information about their offers will be sent exclusively by Anna Guzior-Rutyna.

I am aware that I can withdraw my consent at any time, without affecting the lawfulness of the processing carried out before its withdrawal.
